Route To Transition’s study on homelessness and its effects on young people shows that the number of young people becoming homeless has remained excessively high, reducing the life chances of too many young care leavers. Route To Transition recognises the negative impacts (i.e. risk to health, safety, economic well-being etc) of homelessness, causing increased mental health problems, isolation leading to reduced emotional support, substance misuse, increased sexual activity bringing the risk of pregnancy and of infections, not engaging in education, training or employment, not participating in creative activities.
Route To Transition will assist and look after young people in making a smooth transition from care to independence, and we will provide services that young people will be able to relate to and understand. We provide warm and caring services for young people with emotional, learning, social and behavioral difficulties who face hardships in living with their families. Young people might have to deal with physical abuse, emotional abuse, sexual abuse, neglect, and domestic violence in their past that leads to the risk of absconding, sexual exploitation, offending behaviors, non-engagement in education, and self-harming behaviors. However, we understand that proper care and love can help them rebuild their trust in the community and develop patterns for living a meaningful life.
Our service is targeted not only to prevent social exclusion, but to increase life chances and personal development of all our residents. Over the past few years, we have worked hard to enhance the potential of our services to achieve permanent goals and deliver the best possible outcomes for young people.
