OUR OBJECTIVES

Route to Transition’s objectives with residents are to support and enable them to understand more about themselves, their needs and abilities, learn to manage their behaviour and understand the responsibilities that accompany their move towards independence when they move on.

Therefore, we support residents to learn to abide by the terms of the placement agreement and standards of behaviour of the placement.

This involves encouraging them to:

  • Respect the privacy and property of other residents and the Support Staff
  • Make informed choices about their daily lives and future plans
  • Gain practical skills such as budgeting, cooking, personal cleaning, hygiene, decorating and basic house maintenance
  • Develop positive emotional well-being and good mental health
  • Develop good standards of health and sexual health care, including advice on HIV/Aids and contraception
  • Learn to manage and control their behaviour
  • Develop self-confidence and a belief in their ability to manage their own lives
  • Practice and develop self advocacy skills;
  • Gain some understanding of their life in Care and/or separation from their families
  • Maintain, improve and develop positive relationships with family and other significant people
  • Understand the demands and requirements of parenting and the needs of young people.
  • Work on issues of racial/cultural identity and strategies for dealing with discrimination and adapting to living in the UK
  • Identify ways of using their time productively by exploring with them during support sessions education, training and employment opportunities
  • Build up local networks and knowledge of local resources, through other youth and community projects, leisure services etc
  • Learn about the importance of paying rent regularly and understand the consequences of non-payment
  • Learn to deal with Benefits Agencies and Housing Departments
  • Learn what is involved in moving on into less supported accommodation, or into their own flat when they leave the project
