Route to Transition is a Semi-Independent Accommodation, a Social Enterprise (non-profit) organization and we are at the heart of preventing young people transitioning from statutory care to Homelessness and Unsupported Services. Our story began with the simple idea of serving the community and helping young people who need assistance to build a life of independence and achieve stability and confidence. From that empathetic idea of helping, we have come a long way and supported many young care leavers in achieving a meaningful autonomous life.

We are your trusted providers of semi-independent accommodation and support services for young care leavers. We are a community of homes set up for young people and young people’s service providers who work ethically and efficiently to deliver the best quality outcomes to vulnerable young people and their families.

We provide standalone and staff shared accommodation for care leavers. Also, our team of well-trained and professionally supervised support workers ensures that the needs of the young person are met with utmost empathy. We also make sure that we can provide excellent services and meet the goals of the care leaving people and their responsible authority.

24 hours staff supported living accommodation, self-contained flats, fully furnished with suitable furniture and equipped for supported living with the following items included for Group Living accommodation, some of these rooms and facilities are communal.
Specialized, practical, and most importantly, non-judgmental support, advice, and protection.
Exclusive housing and community support for young people dealing with or at the risk of homelessness
Special services for 16-25 year olds, including supported housing, foyers, and community support.
Caring staff and personalized programs for young people with learning and/or physical disabilities
Wide range of housing and support services for people with diagnosed mental health needs.
